I understand and agree that by signing this waiver I am freeing the Garfield Alumni Foundation and its employee(s) officers, and agents from any liability resulting from, my child(ren) or employee(s) participation in the event or activity. I hereby represent that I understand and am familiar with the nature of the activities in which I (or my child/employee) will participate in the recreational program. I, by accepting below, affirm that I have personally read and understand the release of liability.

I give my permission to the Garfield Alumni Foundation to photograph/record images of myself, children, or employee’s participation in the program for use in Garfield Alumni Foundation publicity and publication and will not seek compensation for such. Any recording will be used for the purpose of promoting various programs and services to the community. I, by accepting below, affirm that I have personally read and understand the release of liability.
